Facility Evaluation Report

On December 4, 2025 San Bruno Regional Office- San Bruno Unit conducted an office/virtual meeting via Teams.

- CCLD Licensing Program Analyst Murial Han

During the meeting, LPA Han reviewed the following serious violations: 80072(a)(3) Personal Rights, 80075(a) Health Related Services, 80078(a) Responsibility for Providing Care and Supervision, 80065(f)(3) Personnel Requirements, 80065(i)(2) - Personnel Requirements and the administrator provided details of the plan of correction that the facility has completed and implemented since the incident.

During the meeting, the Department discussed a Non-Compliance Conference in the near future to discuss in detail of the incident and the plan of correction.

This report is reviewed and discussed with the administrator.
