On 6/16/2025 Licensing Program Analyst (LPA) Murial Han conducted an unannounced visit. LPA met with caregiver, Mark Paul Dela Cruz and LPA explained the purpose of today's visit.
LPA delivered an immediate exclusion letter to exclude a staff (S1) who is no longer working at the facility.
The letter was provided to the caregiver and the administrator was informed over the phone of this letter.
This report is reviewed and discussed, and a copy is provided.
