Facility Evaluation Report
On July 16, 2026, Licensing Program Analyst (LPA) Murial Han conducted a case management visit to follow-up on two incidents that were reported by the facility pertaining to resident #1 (R1). Upon entry, LPA met with caregiver, Karrie and LPA explained the purpose of today's visit.
On June 22, 2026 the facility reported that R1 fell as he/she missed a step from the garage entry into the kitchen resulting in a fall and sustaining an injury on the nose. The incident report indicated that a staff was following R1 when the incident happened but the staff was not able to catch R1.
After the incident, R1 was brought to the hospital, and the facility reported it to R1's responsible party, social worker and CCL.
On July 8, 2026, the facility reported that on July 6, 2026, R1 was brought to the emergency room due to a bruise that was observed on R1's right eyelid. The incident report stated that R1 reported that he/she fell but no additional details were provided. In addition, facility staff reported that on July 4th, 2026, R1 had a witnessed fall and sustained a bruise on the chin.
During today's visit, LPA observed R1 to be pleasant and reported that he/she was not in pain. LPA interviewed staff #1 (S1) who reported that there were no additional falls and they have implemented fall prevention measures such as frequent staff monitoring, encouraging R1 to use to front door entrance instead of the garage entrance to avoid stairs, etc.
