Facility Evaluation Report
On 4/4/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Plan of Correction(POC) visit in regards to a citation given to the facility during the Annual inspection conducted on 3/12/2025. LPA Calandra was greeted by Bienvenido Tabago, Caregiver and explained the purpose of the visit.
On 3/12/2025, LPA Calandra cited the facility for Health and Safety Code 1569.695(c) for Emergency Plans. During the visit, the Licensee/Administrator was unable to provide documentation of their last quarterly emergency drill.
The deficiency has been corrected and cleared.
An exit interview was conducted. This report was reviewed with Bienvenido Tabago, Caregiver and a copy of the report left at the facility.
