On 10/7/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit to follow up on the exclusion of a staff member, S1. LPA Calandra was greeted by Claire Ascalon, Co-Administrator and explained the purpose of the visit.
According to Claire, S1 is no longer a employee as of 8/8/2025. Claire provided a copy of the facility's current LIC 500.
No deficiencies cited during today's visit.
An exit interview was conducted. This report was reviewed with facility representative and a copy provided.
