In response to Incident Report of 6/18/25, LPA Jeung met with RN/administrator to discuss incident, reviewed client file and staff files, and interviewed client. Report of Suspect Elder Abuse (SOC341) is given to LPA. Administrator has conducted an investigation into the incident involving client #1 and staff #1. Investigation report is requested to be submitted to CCLD by Friday 7/11/25.
