On 7/30/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to deliver an Amended version of an original complaint investigation report dated 7/16/2026. LPA Calandra was greeted by Jennifer Duenas, Executive Director and explained the purpose of the visit.
LPA reviewed the Amended report with the Administrator and explained the reason for the Amendment.
No deficiencies cited during today's visit.
An exit interview was conducted and a copy of this report was provided to the Licensee whose signature confirms receipt of the report.
