The inspector’s account

On February 26, 2026, Licensing Program Analyst (LPA) Komal Curley conducted a Case Management-Other visit to the facility. LPA met with Regional Operations Specialist, Caroline Frangieh and explained the purpose of the visit.

The purpose of this visit is to follow up on a "Decision and Order" for the exclusion of Staff 1 (S1) and Staff 2 (2) and revocation of administrator certificate for Staff (3). During the visit, Regional Operations Specialist confirmed that S1-S3 no longer are employed with the facility.

No citations are issued during the visit. Report is reviewed with Operations Specialist, Caroline Frangieh and a copy is provided.
