The inspector’s account

On October 22, 2025, Licensing Program Analyst (LPA) Komal Curley conducted a case management visit in relation to complaint #14-AS-20250113154240. LPA met with Regional Director Specialist, Tammie Sampedro and explained the purpose of the visit.

Based on the course of the investigation, the reported resident checks for Resident 1 (R1) were random or every couple of hours and there was found to be no requirement to document or record when the checks were done, who conducted the checks, or what observations were made. Staff were unable to report any observations based on recall but claim these checks for R1 were conducted.

Deficiencies of the California Code of Regulations, Title, 22 cited on the LIC809-D. Failure to correct the deficiencies may result in civil penalties. Report is reviewed with Regional Operations Specialist, Tammie Sampedro and a copy is provided with appeal rights.
