Facility Evaluation Report
On 8/29/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit in regards to a request to change the Administrator of the facility. LPA Calandra was greeted by Gabriella Johnson, Administrator and explained the purpose of the visit. Tatiana Barsanti, Quality Assurance Manager and Anthony Lupian, Interim Senior Vice President joined the visit later.

No deficiencies were cited during today's visit.
An exit interview was conducted. A copy of this report was left with facility representative.
