On 10/1/2015, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct the 1-year required Annual Inspection. LPA Calandra was greeted by CC DeGraff, Senior Executive Director and April Vargas, Executive Director/Administrator and explained the purpose of the visit.
LPA Calandra reviewed 10 resident files. All were observed to be complete.
The Annual inspection will be completed at a later date.
No deficiencies were cited during today's visit.
An exit interview was conducted. A copy of this report was emailed to the facility representatives.
