The inspector’s account

On 6/26/2026, LPA Grace Donato arrived at the facility to conduct the Annual 1-year required inspection. LPA met Administrator Andrew Frisch, Program Manager Maricel Guevarra and Residential Program Director Andrea Stilleson. LPA explained the purpose of the visit.

LPA toured the facility. This is a 4 building facility with 10 bedrooms, 8 bathrooms, 4 dining rooms, 4 living rooms, a garage used for storage and a gym. All bedrooms had the required furniture and sufficient lighting. No accessible bodies of water or hazards were observed in hallways or the backyard. The facility's fire alarms and Carbon Monoxide detector were observed to be in working order. The facility's hot water temperature was measured 108 degF. The facility had the required 7 days of non perishables and 2 days of perishables on site. The facility was maintained at a comfortable temperature of 70 degF. The facility's fire extinguishers were all checked and updated.

A review of Centrally stored medications indicated that medications for residents were properly labeled with instructions on dosage and times of day and matched the Centrally Stored Medication records kept at the facility.

LPA reviewed 5 resident records and 3 staff files. All were observed to be complete.

No deficiencies were cited during today's visit.
