Facility Evaluation Report
Licensing Program Analyst (LPA) Jaime Vado conducted an announced case management - COVID19 inspection with Program Clinical Consultant (PCC) Cristina Wong and Toni Rivera. This inspection is being conducted a tele-inspection via facetime due to the facility having one COVID positive resident. This resident is not present in the facility as of today's date due to the resident being out of town.
LPA and PCCs met with administrator Andrew Frisch. All residents and staff were PCR tested on 12/27/2021 and the results are negative. One staff member's result has not returned as of yet and this staff person is not working as of today. A tour of the facility was conducted via facetime. It is observed that the facility has required COVID19 signs posted on front door of the facility. PCC advised that staff should be screened by another staff person instead of self screening. Masks, gloves, hand sanitizer, check in log, COVID questions, and touch less thermometer is observed. Adjacent bathroom is observed with liquid soap, hand washing signs, paper towels, and trash can with no lid. PCC advised trash cans with lids as COVID is air born. Dining room is observed with appropriate social distancing in place. Tables are wiped down regularly according to Andy, Commercially prepared cleaning supplies are used. Kitchen is observed with signs posted but hand washing sign is not posted above kitchen sink. Hand washing sign is advised to be posted above or near the sink. Kitchen uses paper towels and no hand towels. Kitchen garbage is also observed with no lid. PCC advised trash can with lid. Andy explained that the lid mechanism has broken on most of the garbage can in the facility but due to heavy resident and staff use the mechanism for the lid to raise up and down has broken. Cleaning supply room is observed as well as additional paper towels. PPE is observed in an adjacent office and additional PPE supplies are stored in an additional adjacent office. PPE supply is approximate to be enough for 60 days. All staff were trained for mask fitment but staff has not been Fit tested as of today's inspection. PCC advised Fit testing of staff. Public Health assisted in donning and doffing of masks and gowns.
Screening sign in sheet needs to include all questions for COVID symptoms
Staff should be screened by another person and not self screening
Hand washing sign to be posted next to or on top of kitchen sink
Fit testing of N95 grade masks for all staff
