On 10/22/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management-health and safety regarding a person who was admitted to the facility. LPA Calandra explained the purpose of the visit to Administrator/Licensee, Stela Calder.
According to the Licensee/Administrator, the resident is no longer living at the facility.
No deficiencies cited during today's visit.
