- Staff did not provide a safe and comfortable environment for resident

On this day Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced complaint investigation visit in order to investigate the allegations received. LPA met with the administrator Ricardo Aban and explained the purpose of this visit.

During the course of the investigation, LPA made observations of the room of R1, locking mechanism of the door of R1, reviewed pertinent documents, and conducted interviews. LPA observed the lock on the door of R1 as not facing out, but rather is lockable from inside the room of R1. LPA checked the door and locking it from the inside. Although the door is lockable from inside the room of R1staff can open it without a key. When observing the room of R1 he/she was in the room and LPA observed him close the door and lock the door on his own. Later during the visit LPA observed him leaving his room and walking around the facility. These allegations are unsubstantiated.

Based on these observations, the above allegations are UNSUBSTANTIATED.

Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the above allegations are unsubstantiated at this time. Report is reviewed with the administrator.
