Facility Evaluation Report
On 7/9/2026, LPA Grace Donato conducted an unannounced annual visit to the facility. LPA was met by Kendra Ceccato, Associate Program Director and Sayoko Yoshimura, Program Director and explained the purpose of the visit.
LPA toured the facility. This facility is composed of two separate buildings, one that has administrative offices, the kitchen, pantry, dining room, living room, and the second building has the facility's laundry room, garages, and client bedrooms and bathrooms. All bedrooms had the required furniture and sufficient lighting. No accessible bodies of water or hazards were observed in hallways or the backyard. The facility's fire alarms and Carbon Monoxide detector were observed to be in working order. The facility had the required 7 days of non perishables and 2 days of perishables on site. The facility was maintained at a comfortable temperature of 70 degrees Fahrenheit. The facility's fire extinguishers were observed to be fully charged.
LPA reviewed 5 resident records and 5 staff records. All were observed to be complete.
A review of Centrally stored medications indicated that medications for residents were properly labeled with instructions on dosage and times of day and matched the Centrally Stored Medication records kept at the facility.
No citations were issued during today's visit.
