Facility Evaluation Report
On 5/15/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit follow up on an incident reported to the Department on 5/12/2025 regarding a resident (R1) who was accused of assaulting another resident (R2). LPA Calandra was greeted by Kendra Ceccato, Assistant Program Director and Sayoko Yoshimura, Program Director and explained the purpose of the visit.
LPA conducted interviews and gathered and reviewed the following documents for R1:

Based on document review and interviews, it was learned that on 5/9/2025, R2 alleged that R1 assaulted them. R1 had lived at the facility for nearly 6 months and no other similar incidents have been reported. R1 is not currently a resident at the facility as of 5/12/2025. R2 will be moving back to the facility soon from the hospital, and Redwood City Police will be investigating.
No deficiencies were cited during today's visit.
An exit interview was conducted. This report was reviewed with Kendra Ceccato, Assistant Program Director and Sayoko Yoshimura, Program Director.
