Facility Evaluation Report

On 8/8/2025 Licensing Program Analyst (LPA) John Calandra conducted an unannounced Case Management visit for the purpose of delivering an immediate exclusion letter for S1. LPA met with nurse, Hegay Bhonjatsaag, LVN. Hegay contacted the administrator who arrived later during the visit. LPA explained the purpose of today's visit to Administrator, Madonna Valencia.

LPA delivered an immediate exclusion letter to exclude staff (S1) and according to Administrator ,S1 will no longer work in the facility and will be disassociated in Guardian. Staff was advised that S1 is not allowed in the facility.

LPA reviewed the immediate exclusion letter.

No deficiencies were cited during today's visit.

This report is reviewed and discussed with caregiver, and a copy is provided.
