On November 15, 2024, Licensing Program Analyst(LPA) John Calandra arrived at the facility at 1:30 PM to complete the Annual 1-year required inspection. LPA Calandra was greeted by Elenita "Ruby" Cruz, Caretaker and explained the purpose of the visit. Evelyn Ryan, RN, Licensee/Administrator and Corazon Medina, Lead Staff arrived later during the visit.
LPA Calandra reviewed 2 resident files. Both were observed to be complete.
No deficiencies were cited during today's visit.
