On 5/8/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit to deliver the facility's new license. LPA Calandra was greeted by Maria Collins, Licensee and explained the purpose of the visit.
A new license was provided as the Licensee's Hospice waiver was increased from 2 to 3 persons.
No deficiencies cited during today's visit.
An exit interview was conducted and a copy of the report provided to the facility representative.
