The inspector’s account

On 8/5/2026, Licensing Program Analyst(LPA) Grace Donato arrived at the facility to conduct the Annual 1-year required inspection. LPA was greeted by Staff Emma Advincula, and explained the purpose of the visit. Administrator, Cynthia Advincula arrived later during the visit.

LPA toured the facility. This is a one-story building with 8 bedrooms, 2 bathrooms, living room, kitchen, and dining room. All bedrooms had the required furniture and sufficient lighting. Both bathrooms had anti-skid floor mats and grab bars. The fire place in the living room is covered. The smoke detector and carbon monoxide detector were observed to be in working order. First Aid kit is complete. The facility's hot water temperature was measured 106 degF. The facility had 7 days of non-perishables and 2 days of perishables on hand. All sharp objects, detergents, poisons, and soap were observed to be locked and in-accessible to persons in care.

LPA reviewed 4 resident records and 2 staff files. All records are complete. LPA reviewed medication, everything is complete and updated.
