Facility Evaluation Report
On 2/4/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management regarding an incident involving missing Personal and Incidental (P&I) money of R1. LPA Calandra was greeted by JR Loreto, House Manager and explained the purpose of the visit. Sally dela Cueva, Co-Administrator arrived later during the visit.
According to the House Manager, they were putting away P&I money for another resident away in the lock box and found P&I money for R1 to be missing. The Licensee is currently investigating the matter and has
