Facility Evaluation Report

On 9/29/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit in regards to the Licensee reporting to the Department that residents were relocated temporarily. LPA Calandra was greeted by Justine Galera, Direct Support Professional(DSP) and explained the purpose of the visit.

LPA toured the physical plant. This is a 1-story building with 3 bedrooms, two bathrooms, a kitchen, living room, dining room, garage, and front and back yards. All bedrooms had the required furniture and sufficient lighting. The facility had the required 2 days of perishables and 7 days of non perishables on site. The Licensee had a first aid kit with the required items.

All sharp objects, medications, soap, detergents, and poisons were observed to be locked and in-accessible to persons in care.

No deficiencies were cited during today's visit.
