This is an official report of an unannounced visit/investigation of a complaint received in our office on
01/06/2021 and conducted by Evaluator Jaime Vado
COMPLAINT CONTROL NUMBER: 14-AS-20210106160408
R1 was left sitting on the toilet for long periods of time without any assistance
Facility is not following dietary needs of the reisdent based on his condition
Staff ignored the clients calls for help so R1 had to call Fire Department for assistance
On this day, Licensing Program Analyst (LPA) conducted an unannounced complaint investigation visit in order to deliver the findings regarding the allegations. LPA met with administrator Monica and explained purpose of today's visit.
During the course of the investigation interviews were conducted and review of pertninent documents were made. Due to the COVID pandemic at the time of the allegations several staffing impacts took place in regards to staffing changes, temporary agency use utilization, and multiple staff leaving, the stafffing levels fluctuated within the facility this was something the facility was not able to control due to the pandemic. Staff tended to residents the best they could to meet residents needs. At no point was it found that there were no staff present. These allegations are unsubstantiated.
Based on these observations, the above allegations are UNSUBSTANTIATED.
Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the above allegations are unsubstantiated at this time.
