Facility Evaluation Report
On March 12, 2026, Licensing Program Analyst (LPA) Komal Curley conducted an unannounced case management to follow up on a visit that was conducted on 2/20/26. LPA met with Administrator, Osyuki Herrera and explained the purpose of the visit.
Based on interviews and record review, a sexual incident occurred on 12/23/25 between Client 1 (R1) and Client 2 (R2). This incident was not reported to CCLD until 1/15/26 via email to LPA Curley. On 1/16/26, LPA Curley notified Administrator to fax the incident reports to the Regional Office, however Regional Office did not receive the incident reports until 1/22/26. The facility did not report the incident to R1's conservator until 1/9/26, did not report to LTCO until 1/12/26, and did not report to police department until 1/14/26.
The facility failed to report CCLD in a timely manner.
During case management visit regarding an unusual incident, A deficiency was cited per California Code of Regulations, Title 22. See LIC809-D. Failure to correct the deficiencies may result in civil penalties.
Report is reviewed with administrator and a copy is provided.
