Facility Evaluation Report

On February 20, 2026, Licensing Program Analyst (LPA) Komal Curley conducted an unannounced case management visit in relation to two incident reports that were submitted to CCL on 1/22/26. LPA met with Administrator, Osyuki Herrera and explained the purpose of the visit.

On 1/22/26, the Licensee reported, on 1/14/26, during a routine interview with Client 1 (R1) disclosed there was a sexual encounter with Client 2 (R2) that initially was determined as consensual, however is now being reported as non-consensual. Police Department, LTCO and responsible parties were notified.

During the visit, LPA reviewed R1 and R2's file, reviewed internal documentation, interviewed the administrator and obtained information in relation to the incident. According to the administrator, R2 is no longer a client at the facility. Based on documents reviewed, Staff 1 (S1) did observe the incident.

Further investigation is required. Report is reviewed with Administrator and a copy is provided.
