The inspector’s account

On 8/22/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Plan of Correction(POC) visit to clear the citation delivered on 8/15/2025. LPA Calandra was greeted by Ludy Pantaleon, Caregiver and Nelia Asuncion, Administrator/Licensee and explained the purpose of the visit.

On 8/15/2025, the facility was cited for a violation of Health and Safety Code(HSC) 1503.2-General Provisions as the Licensee did not have a functioning Carbon Monoxide detector at time of visit. On 8/22/2025, LPA verified that a Carbon Monoxide detector has been purchased and installed in the facility.

On 8/15/2025, the facility was cited for a violation of California Code of Regulations(CCR) 80075(f) Health Related Services as the Licensee did not have any staff working with active CPR training. During his visit on 8/22/2025, LPA verified and received copies of both Licensee and Caregiver, Ludy Pantaleon's CPR training that was completed by the Plan of Correction(POC) due date.

On 8/15/2025, the facility was cited for a violation of HSC 1565 Other Provisions(aka Emergency Drills) as the Licensee had not conducted a Emergency drill since March of 2024. Emergency Drills shall be conducted on a quarterly basis. On 8/22/2025, LPA Calandra was able to verify that an emergency drill was conducted prior to the Plan of Correction(POC) due date.

An exit interview was conducted. A copy of the report and the POC clearance letter were left at the facility with the facility representative.
