On 8/8/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility at 1:30pm to conduct the Annual 1-year required inspection. LPA Calandra rang the doorbell twice and knocked but no one answered the door. LPA Calandra looked through the window but did not observe any staff or clients. This visit is recorded as attempted per FAS guidelines. Facility representative's signature could not be obtained.
The Annual inspection will be completed at a later date.
