The inspector’s account

On 10/15/2025, Licensing Program Analysts (LPA) Grace Donato & Komal Curley, Licensing Program Manager (LPM) Brenda Chan, Oakland Regional Manager for Adult and Senior Care (RM) Isaac Taggart and Supervising Special Investigator Megan Mullen conducted an unannounced Case Management visit.

The purpose of the visit was to deliver a Temporary Suspension Order (TSO) effective October 15, 2025 at 5:00pm to the facility licensees, Cesar & Mayra De Jesus Herrera. The TSO notified the Licensees that the facility will be closed and the residents will be relocated.

Representatives from law enforcement were present during visit as well.

A TSO was issued and Accusation and Notice of Defense were explained.

Deficiencies were cited as per California Code of Regulations Title 22. See LIC809-D for more information.
