Facility Evaluation Report
On 10/12/2025, Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced case management - health and safety check in response to an incident that occurred on 09/30/2025 and subsequent complaint allegations received. LPA met with co-administrator Mayra Herrera. Currently there are 4 clients in the facility and 4 clients are out with their family members. Two staff are present and sister in law of the family is present. She is fingerprint cleared and associated.
During today's visit LPA toured the physical plant of the facility and spoke to staff. LPA observed residents and facility in regards to health and safety. Residents and facility appear to be clean, safe, and in place. Resident is observed conducting weekend chore/goal is to water the garden in collaboration with the day program to meet those goals by watering the plants. Resident rooms are clean and in place. Facility is clean and in order. Facility ambient temperature is comfortable. Backyard is secured and clean. Facility does have dogs in the backyard but dogs are not aggressive. LPA observed no other staff or family members present in the facility besides the two staff persons Mayra Herrera Lima and her mother/staff Mayra Herrera.
LPA received during today's visit an updated LIC308 and POC for citation issued on 10/10/2025.
