Facility Evaluation Report
On 10/11/2025, Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced case management - health and safety check in response to an incident and subsequent complaint allegations received. LPA met with co-administrator Mayra Herrera. Currently there are 5 clients in the facility and 3 clients are out with their family members. Two staff are present.
During today's visit LPA toured the physical plant of the facility and spoke to staff. LPA's purpose is to identify resident rooms and staff rooms. LPA observed residents and facility in regards to health and safety. Residents and facility appear to be clean, safe, and in place. Resident is observed conducted weekend chore/goal is to water the garden in collaboration with day program to meet goals. Resident rooms are clean and in place. Facility is clean and in order. Food supplies are observed and are in place. LPA observed no other staff or family members present in the facility besides the two staff persons Mayra Herrera Lima and her mother/staff Mayra Herrera.
LPA received updated copy of facility floor plan and facility resident roster.
Report is reviewed with Mayra Herrera Lima and a copy is provided.
