On June 16, 2026, Licensing Program Analyst (LPA) Murial Han conducted an unannounced Case Management-Other visit. LPA met co-administrator, Loi Bautamante and explained the purpose of today's visit.
LPA delivered an immediate exclusion letter for staff #1 (S1). The co-administrator confirmed that this staff no longer works at the facility.
The exclusion letter was provided to the co- administrator.
