The inspector’s account

On October 7, 2025, Licensing Program Analyst (LPA) Komal Curley conducted an unannounced annual inspection. LPA met with Program Director, Dao Do and explained the purpose of the visit.

LPA toured the facility including all of activity rooms & common areas. The indoor and outdoor passageways were free of obstruction. Participants were observed participating in activities. A comfortable temperature is maintained and lighting is sufficient for comfort. Delayed egress was observed to be in good working condition. Facility has a sprinkler system. All fire extinguishers have been checked and current. Client bathrooms were observed to be in good repair. Water temperature throughout the facility was measured between 117-120 degrees F. Chemicals, sharps and toxins were observed locked in the kitchen and inaccessible to clients. According to the Program Director, meals are still being provided by Meals On Wheels program and brought to the facility in the morning. Staff warms the food in the kitchen area prior to serving lunch. Medications were observed in a locked container in the nurse's room inaccessible to clients. Door to the nurse's room is in a locked area which is only accessible by staff.

LPA reviewed five client records and five staff records were reviewed. Resident records are updated, complete and signed. Staff records are complete.

No deficiencies are issued during the visit. Report is reviewed with the Program Director and a copy is provided.
