Facility Evaluation Report
On July 8, 2026, Licensing Program Analyst (LPA) Murial Han conducted an unannounced case management visit to follow up on a 30-day eviction letter that the facility issued to resident #1 (R1). LPA met with Administrator Designee Jizeel Clement and Quality Assurance, Jennifer Tobias.
On July 6, 2026, Ms. Tobias provided a copy of R1's 30-day eviction notice to LPA that was provided to R1's responsible party. After reviewing the 30-day notice, LPA informed Ms. Tobias that the 30-day notice was not in compliance with Title 22 under Eviction Procedures and it needed to be retracted, revised and reissued.
During today's visit, LPA met with Ms. Tobias, and reviewed the Title 22 Regulation for Eviction Procedures. Ms. Tobias stated that the facility will retract the 30-day eviction notice and will reissue it to the responsible party.
This report is reviewed and discussed with the Administrator Designee, Quality Assurance and Resident Care Coordinator.
