Facility Evaluation Report
On December 4, 2025, Licensing Program Analyst (LPA) Komal Curley conducted an unannounced annual visit. LPA met with Assistant Program Director, Dabatha Sequeira and explained the purpose of the visit.
LPA toured the facility inside and outside including, activity rooms, common area & kitchen. The indoor and outdoor passageway was free of obstruction. No accessible bodies of water of fire safety hazards observed. This is a two story facility. LPA observed 4 activity/classroom areas and main activity room to be free from tripping hazards and spacious. Kitchen was observed to be locked. According to Assistant Program Director, clients bring their own lunches to the day program and are able to store their lunches in the kitchen fridge. LPA observed separate bathrooms for male and female. Bathrooms were observed to be clean, odor- free and equipped with paper towels and liquid soap. Water temperature throughout the facility measured between 115-116 degrees F. Sharps and chemicals were observed locked an inaccessible. There are currently no clients taking medication at the facility.
The second floor of the facility was observed to have a computer lab and a conference area. Outside patio area was observed shaded and free from tripping hazards. During the visit, LPA observed clients eating their lunches outside. A comfortable temperature is maintained throughout the facility and lighting is sufficient for comfort. Carbon monoxide monitors are working properly. All fire extinguishers have been checked and current as of November 2025. Emergency drills are logged and done every month. LPA reviewed 5 client records and 5 staff records. Client records are updated, complete and signed. Staff records are complete, with training logs that have met the basic requirement.
No citations are issued during the visit. Report is reviewed with Assistant Program Director and a copy is provided.
