Facility Evaluation Report

On 10/23/2025, Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced, case management health check, to check on a client that was recently relocated from another facility. LPA met with staff person Lui Yolangco and explained the purpose of today's visit.

According to Lui, the client (C1) is not at this facility. C1 never was sent to this facility as C1 is currently at their sister facility Caring Hearts Home #2, facility number 410508749 , located in Daly City. Lui also works at Caring Hearts Home #2 and confirmed that he is at that location. Per information received regarding the location of C1, LPA confirmed that incorrect information on the location was provided. The client is not at this facility. No inspection or other observations made as a result.

Report is discussed with Lui and a copy is provided on this day.
