On 2/2/23, Licensing Program Analyst (LPA) Komal Charitra conducted an unannounced visit and met with the Administrator, Freddie Fullon and explained the purpose of the visit.
The purpose of today's visit is to deliver an immediate exclusion letter to exclude an employee of the facility.
The letter was given and reviewed by the administrator.
