Facility Evaluation Report
On 9/5/2025, Licensing Program Analyst (LPA) John Calandra arrived unannounced for the purpose of conducting a Case Management, Health & Safety visit. LPA Calandra met with Administrator (ADM), Fe Arnaiz and explained the purpose of the visit. Administrator, Zach Pilkerton arrived later during the visit.
LPA Calandra toured the facility with the administrator, Fe Arnaiz. The facility was found to be clean and at a comfortable temperature with all exits and doorways free from obstruction. Eight staff members including the Administrators were observed throughout the facility. Twelve residents were observed throughout the facility. LPA inspected random resident bedrooms and resident common spaces.
LPA reviewed 3 resident's Centrally Stored Medications Records. All medications for the 3 residents were properly labeled with instructions on dosage and times of day and matched the Centrally Stored Medication records kept at the facility.
LPA requested the following document by 9/15/2025:
Copy of facility sketch showing where surveillance cameras are in the facility
No deficiencies cited during today's visit. A copy of the report was provided to the facility representatives.
