Facility Evaluation Report
On 3/5/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit in regards to a lack of supervision incident reported to the Department. LPA Calandra was greeted by Abby Carvajal, Program Director and explained the purpose of the visit.
According to the Regional Director of Operations, Tiffany Spiecker it was reported to the Licensee, that R1 had gone on various unsanctioned outings into the community with S1 that were not part of R1's program plan. According to R1's program plan, a 1:1 caregiver was needed. In addition, S1 left R1 alone in the care of an unknown individual, R1 had met online for an unknown period of time. S1 is currently on Administrative leave and will no longer be working for the facility. A Type A citation was issued due to S1's neglect/lack of supervision actions.
During the visit, LPA Calandra collected copies of the facility's LIC 500, Staff schedule, R1's appraisal of needs and services, resume and application of S1. The facility will send a copy of S1's termination letter by 3/6/2026.
Deficiencies are cited under the California Code of Regulations, Title 22. Failure to correct said deficiencies by the Proof of Correction(POC) due date may result in additional civil penalties.
An exit interview was conducted. A copy of this report along with Appeal Rights were provided.
