The inspector’s account

On March 13, 2026, Licensing Program Analyst (LPA) Komal Curley conducted an unannounced annual visit. LPA met with Program Director, Alejandra Duenas and explained the purpose of the visit.

LPA toured the facility inside and outside including, activity rooms, common area & kitchen, bathrooms and dining room. The indoor and outdoor passageway was free of obstruction. No accessible bodies of water of fire safety hazards observed. During the visit, LPA observed clients participating in activities with adequate supervision. Activity rooms and main room were observed to be free from tripping hazards and spacious. Kitchen was observed to be clean and odor-free. According to Program Director, clients bring their own lunches to the day program and are able to store their lunches in the kitchen fridge or the staff will bring clients to go buy lunch. LPA observed two single private bathrooms and one bathroom with two stalls. Bathrooms were observed clean and in good repair. Water temperature throughout the facility measured between 114-115 degrees F. Sharps and chemicals were observed locked an inaccessible. There is currently one client (R1) who does administer his /her own medication and performs his/her own glucose testing. Staff do not assist R1 with medication or glucose testing.

A comfortable temperature is maintained throughout the facility and lighting was sufficient for comfort. First aid kit was observed to be present. Bathrooms were observed to be in good repair. Emergency drills are being conducted and logged once a month. Carbon monoxide detectors were observed to be in working condition. LPA checked all fire extinguishers which were inspected on August 2025. LPA reviewed 5 client files and 5 staff files.

Deficiencies were cited per California Code of Regulations, Title 22. See LIC809-D. Failure to correct the deficiencies may result in civil penalties. Report is reviewed with Program Director and a copy is provided.
