Facility Evaluation Report
On 9/4/2025, LPA Grace Donato made an unannounced annual visit to the facility. LPA met with Case Manager, Angie Jacob and LPA explained the purpose of the visit.
LPA toured the facility including all of activity rooms & common areas. The indoor and outdoor passageways were free of obstruction. Participants were observed in the main common area, engaging in activities, exercising, and assisting with daily chores. While touring the facility it was observed that the room temperature was at 68 deg F. Facility has a sprinkler system. All fire extinguishers have been checked and current. Client bathrooms were observed to be in good repair. Food are brought in by residents prepared from their home facilities. Emergency exit routes are marked, and an emergency evacuation plan was posted in each room. Emergency drills are done every
Five client records and four staff records were reviewed. Resident records are updated, complete and signed. Staff records are complete.
