On 3/5/2026, LPA Grace Donato conducted an unannounced Case Management-Other visit. LPA met with Case Manager Angie Jacob and explained the purpose of the visit.
LPA delivered an immediate exclusion letter for a staff member who is currently associated in the facility.
The letter was given to and reviewed by the Case Manager.
