Facility Evaluation Report
On March 2, 2026, Licensing Program Analyst (LPA) Komal Curley conducted an unannounced case-management visit to follow up on a visit that was conducted on 2/12/26. LPA met with Administrator, Ryan Mock and Program Director, Angie Jacob and explained the purpose of the visit.
On 2/12/26, LPA Jeung conducted a case-management visit in response to an incident report that was submitted to CCLD on 2/10/26. The Licensee reported, on 2/10/26, Client #1 (R1) suddenly engaged in an aggressive behavior towards Staff #1 (S1). In an attempt to stop R1's behavior, S1 held R1 by the neck. Staff was immediately removed from facility on 2/10/26 pending investigation. During the visit on 2/12/26, staff interviews were conducted and documents were reviewed.
During the visit today, LPA obtained additional information in relation to the incident. Training records were reviewed and additional documents were collected. Camera footage observed confirmed that S1 did hold R1 by the neck. According to the Administrator and Program Director, S1 was terminated.
A deficiency was cited per California Code of Regulations, Title 22. See LIC809-D. Failure to correct the deficiencies may result in civil penalties.
