On 2/27/2026, LPA Grace Donato conducted an unannounced Case Management-Other visit. LPA met with Case Manager Angie Jacob and explained the purpose of the visit.
LPA delivered an immediate exclusion letter for a staff member who is currently associated in the facility.
LPA also provided the letter which is addressed to the staff. The letter was also explained to the staff. Staff was asked to leave the facility after delivery of letter.
The letter was given to and reviewed by the Case Manager and Staff.
This report is reviewed and discussed, and a copy is provided.
