On this day at 0930hrs, Licensing Program Analyst (LPA) Jaime Vado and Komal Charitra conducted an unannounced case management inspection visit to discuss and obtain documents pertaining to R1. LPAs met with administrator Karen Nickolai and explained purpose of today's visit.
LPAs discussed with Karen the circumstances regarding the eviction of R1. LPAs requested specific document during today's visit and received some items in person today. Additional items requested will be forwarded to LPA Vado to LPA once received from facility's corporate office.
