Facility Evaluation Report
On 5/30/2025, Licensing Program Analyst Grace Donato conducted an announced facility closure inspection visit. LPA met with Administrator Marcie Barber.
Licensee has submitted the letter of closure on 5/29/2025.
LPA toured the facility. Facility has two floors and residents resided on both floors. There is one resident room on the 1st floor and two resident rooms on the 2nd floor. LPA toured the facility and it was observed that all residents have been moved. Facility will be cleaned out.
Deficiencies are also delivered during the visit and cited from the California Code of Regulations, Title 22. See LIC809-D. Failure to correct the deficiencies may result in civil penalties.
CCLD will be proceeding with facility closure as the licensee has lost control of property.
A forfeiture letter will be sent to administrator/licensee.
