Facility Evaluation Report
On April 25, 2025, Licensing Program Analyst (LPA) Komal Charitra conducted an unannounced case management visit. LPA met with Program Director, Angel Carter and explained the purpose of the visit.
On April 7, 2025, the Regional Office was notified by Department of Health Care Services (DHCS) that an on-site program certification review was completed on March 24, 2025 and the facility was found to be out of compliance with, California Code of Regulations (CCR), Title 9, Article 3.5, Sections 531 through 533.
According to DHCS, the facility was unable to provide documentation to show that they were providing and documenting at least twenty hours of in-service training for employees.
During the visit conducted today, LPA reviewed 5 staff files and observed all staff training to be current and meeting basic requirements. As of 2025, staff are still working on meeting the 20-hours of continuing education. According to the Program Director, there was one staff member who did not meet the 20 hours of continuing education in 2024, however Program Director implemented a plan of correction to ensure all training with staff are monitored monthly to ensure compliance with CCL and DHCS.
No citations are issued during this visit. LPA reviewed report with Program Director and a copy is provided.
