On October 31, 2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced Case Management-Other visit. LPA met with caregiver, Arsenia Villadarez and LPA explained the purpose of today's visit. Caregiver informed the administrator, Karin Lastimosa via phone of LPA's visit.
LPA delivered an immediate exclusion letter for a staff member who is currently associated in the facility. The administrator confirmed that this staff does not work at the facility.
The exclusion letter was provided the caregiver, Arsenia Villadarez.
