Facility Evaluation Report
On May 11, 2026, Licensing Program Analyst (LPA) Komal Curley conducted an unannounced case management visit in relation to an incident that occurred on 5/4/26. LPA met with Administrator, Nina Aliason and explained the purpose of the visit.
The Licensee reported on 5/4/26, Resident 1's (R1's) day program called the administrator to report that R1 reported to the day program staff that Staff 1 (S1) inappropriately touched him/her. R1 reported to the day program staff that this allegedly occurred before. All required parties were notified.
During the visit, LPA interviewed S1, administrator, and obtained copies of documents.
No citations are issued during the visit. Report is reviewed with Administrator and a copy is provided.
