On October 31, 2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced Case Management-Other visit. LPA met administrator, Renz Corrales and explained the purpose of today's visit.
LPA delivered an immediate exclusion letter for a staff member who is currently associated in the facility. The administrator confirmed that this staff no longer works at the facility.
The exclusion letter was provided to the administrator.
